Workers’ compensation benefits are dependent, by state law, on timely reporting. Additionally, best outcomes for an injured employee are achieved when a worker compensation claim is initiated without delay. All employees shall promptly report any injury that occurs at work as early as possible and, if possible, submit the workers compensation accident form within the assigned shift but no later than 30 days after the injury. Occupational illnesses that develop over time should be reported at the time the worker suspects their symptoms may be work-related. The Workers Compensation accident form can be obtained from your Principal, Immediate Supervisor or the Payroll/Business Office or click on the link below. Once you have completed the form, please submit the completed form to your supervisor.
Failure to report promptly negatively affects the District’s rate and workers compensation benefits may be denied.
